It looks like it may be a dense basalt in the typical style in Arizona north of the Mogollon Rim from about AD 1000 – 1500.

Ones like this (made from non-vesicular dense volcanic material) are typically made at the source by specialist communities.  The volcanic areas near Flagstaff are one such place that produced axes that ended up hundreds of kilometers away.
